New Patient Registration

NHS_Hospital_formThe doctors welcome new patients who live within our practice area (Bury). 

Drop into the Practice and pick up a registration form. As part of the registration process you will be asked to attend for a new patient check.

Security of your medical record is important to us, and you will therefore need to bring:

  • Proof of identity such as your passport, photo card driving license etc.
  • Your NHS medical card from your previous NHS doctor. If you do not have this because you have lost it or because you have recently arrived in the country we can still process your registration but it may take longer to trace your medical records and you will need to fill in more paperwork

You can register online. Click on the Registering Online tab above. You will need to do this for yourself, and any other members of your family which also want to register.

When we receive your details we will complete the paperwork for you and contact you for your signature and to arrange registration.

We cannot accept registrations from patients who live outside the Bury PCT area.
